As an ASC Medical Records Inpatient/Outpatient Coder II, I process all reports of numerical counts of clinical visitations of patients on a daily basis. I will analyze and abstract all procedures and diagnosis that are documented. I would re-sequence all diagnostic codes to the highest coding ability. I read and review the provider’s notes that are documented in E.H.R. Then, key enter the assigned classifying codes using ICD-9, CPT and HCPCS Codes into the RPMS, E.H.R., and the 3M Grouper System. I communicate with nurses or doctor’s if there is any discrepancies’ in the documentation of their notes entered in E.H.R. These will be corrected and I will implement the accurate codes for proper reimbursement. After I have key entered all diagnosis and procedure codes, I will review, complete and close out the visits then forward to billing. I’m experienced in supervising all clinical coders when appointed as the Acting Supervisor when the supervisor is out of the office for a specified amount of time. I will oversee all clinical coders and attend meetings when needed to. Communication with all clinical staff is required on a daily basis. I have the ability to code these clinics: Emergency Department, Pediatrics, Family/Internal Medicine, ENT-Eye Clinic, Elderly, HBC clinic, Rehabilitation Therapy, Dental, Podiatry, Surgical, Woman’s Clinic, Pharmacy, Radiology and Respiratory
